Grant Description

This grant opportunity provides one-off funding of $10,000 per eligible application in the 2024-25 financial year to assist Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP) providers with IT system upgrades necessary for meeting new reporting obligations under the Support At Home Program and the new Aged Care Act.
